​​​ ​​​​​What Is the Long-Term Care Data Cooperative?

The LTC Data Cooperative gathers clinical Electronic Health Record (EHR) data from Skilled Nursing Facilities and Assisted Living, for:
  • Health care operations (feedback reports to providers)
  • Public health surveillance
  • Research on effectiveness
  • Clinical trials
Participating providers receive community- level reports with EHR-based metrics and comparative data as a resource to help with insights on care delivery and EHR documentation.

Participation is free, because this effort is funded by the National Institute on Aging. In addition to the benefits of community-specific reports, by joining, providers securely contribute EHR data to a nationwide learning platform that advances research on the most effective treatments and care practices for older adults living in skilled nursing and assisted living settings.

The Long-Term Care Data Cooperative helps skilled nursing and assisted living providers unlock the value of the data they already collect every day.

This provider-led effort is governed by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). A list of participants who serve on governing committees can be found here.

Provider Reports That Support Better Decision-Making with EHR-based Measures For New Insights


Participating providers receive exclusive reports that transform routine EHR data into meaningful clinical and operational insights. These reports help organizations identify trends, monitor documentation and care practices, support quality improvement initiatives, and better understand resident populations. With explanations of why each EHR-based measure is important to care delivery, the reports make it easier to turn data into action.

What Will Be Collected?


Did you know that LTC Data Cooperative analysts are available to query clinical data from a facility or organization’s electronic health record (EHR) in the LTC Data Cooperative? The LTC Data Cooperative can be a source of data for a Quality Award application or to track outcomes for an internal initiative.

If you have questions about your resident population, we may be able to help answer them with data from your EHR. The data from the EHR we are likely to have include:

  • Resident demographics (including length of stay and admission reason)
  • Vital signs
  • Orders for advance directives
  • Diagnoses
  • Lab orders
  • Lab results
  • Medication administrations
  • MDS assessment items
  • Vaccination records
  • All other orders (therapy, diet, radiology)

We can select a group of residents and look at their care over time. We are also happy to talk with you about possible questions you might benefit from answering with these data.
