Provider Reports That Support Better Decision-Making with EHR-based Measures For New Insights
Participating providers receive exclusive reports that transform routine EHR data into meaningful clinical and operational insights. There are building-specific feedback reports for both skilled nursing centers and assisted living communities (see examples below).
How can providers use these reports?

Compare performance. See how your results compare with other LTC Data Cooperative providers and bring areas of concern to your QAPI team or in other quality improvement processes.

Spot trends. Track whether measures are improving, holding steady, or worsening over time.

Improve EHR documentation. Identify information that may be recorded inconsistently or outside designated EHR fields, where staff and reporting tools may miss it.

Support advance care planning. Check whether advance directive orders are current and easy for staff to find. Explore advance care planning resources from the National Institute on Aging.

See potential issues earlier. Use EHR data to monitor unadjusted trends in hospitalizations, antipsychotic medication use, and weight loss.
